  • Adult Companion Specialist

    Definition:
    Adult Companion Specialist supports cover services that help a person work toward a therapeutic or community integration goal in their support plan.
    The adult Companion Specialist may only assist or supervise the person with tasks such as laundry, light housekeeping, meal preparation and shopping while the companion helps the person work toward a therapeutic or community integration goal.

    Examples
    The adult companion may:
    • May shop with the person to practice budgeting skills.
    • Attend a movie with the person to practice coping skills to manage their social anxiety
    • Go with the person to a community event to reduce their social isolation
    • Play a board game with the person to enhance their fine motor skills
    • Provide verbal instructions or cues to the person to help them complete a task

  • Direct Support Professional

    Job Summary:
    The Direct Support Professional (DSP) performs all duties outlined in this job description. The DSP is an extension of the Designated Coordinator and will provide direct support to the clients we serve. The DSP will work in concert with the Coordinator to ensure that internal policies and external regulations are adhered to.

    Reporting Relationship:
    The DSP reports to the Designated Coordinator.

    Essential Duties and Responsibilities:

    Client Support:

    • Treat clients with dignity and respect in all interactions, even if it is perceived that the client is being disrespectful or attempting to engage staff in an argument or altercation. Never is it appropriate for staff to engage in arguments with clients or to use disrespectful or confrontational language with a client. Remember that the staffs are the professionals, and we are in their homes.
    • Incorporate a client-centered philosophy
    • Accept delegation of various responsibilities per the Coordinator
    • Mediate client crisis situations
    • Ensure that the house program schedule is being followed accurately
    • Ensure that all appointments and community activities are complete
    • Provide client supervision per the client’s Individual Abuse Prevention Plan (IAPP)
    • Administer client medications and complete required documentation
    • Ensure that medication is being ordered and logged in by the assigned staff member
    • Assist clients with budgeting and purchases, ensuring that receipts are obtained for all transactions that clients are recording all transactions in their check ledgers, and that accounts are balanced.
    • Ensure that DSP checklists and overnight checklist are being completed accurately
    • Role model for new staff – be reliable, on-time, disciplined, show follow through, maintain a positive attitude, and support management. If you disagree with a decision made by management, address it with human resources and do not discuss with other employees or clients.
    • Never share negatively feeling about management or management decisions, with clients.
    • Never discuss personnel or employee discipline matters with clients, even if you disagree with management
    • Communicate concerns from clients/legal representatives to the Designated Manager
    • Carry out requests and preferences of guardians and case managers as directed by your supervisor or the client’s team.
    • Always supervise clients closely and according to their Individual Abuse Prevention Plan (IAPP). Never leave a client unsupervised, unless you are certain they are able to be unsupervised. If you are unsure, always ask FIRST before leaving the client. This is true, even if the client says that they can be unsupervised.
    • Never leave a site/program without staff coverage, even if the next shift does not show up. If you leave your shift and the site without staff, regardless of the reason, your employment will be terminated and a report will be made to the Department of Human Services, which may disqualify you from working with vulnerable adults or children anywhere in the United States, indefinitely.
    • Respond to client behavioral and medical crisis immediately and according to policy
    • Attend all mandatory staff meetings
    • Because some of clients have histories of drug and/or alcohol abuse, it is never appropriate to discuss drug or alcohol use or to have drugs or alcohol in your possession while at work. It is our job to support our clients’ sobriety – not to endanger it.
    • Read client files and the support/supervision needs of each client you are charged with supporting
    • Maintain professional boundaries with clients and teams at all times – staff should never talk about personal matters with clients, such as weekend plans, family issues, financial problems. Remember that you are not the client’s friend. You are their staff, who are there to support them and help them be safe and successful
    • Provide daily support for clients with decision making and problem solving.
    • Ensure that the house is kept clean and tidy at all times
    • Ensure that client hygiene is attended to – nails trimmed, foot checks done, showers, clean clothes, etc.
    • Ensure the follow through with all scheduled medical appointments
    • Report any maintenance concerns promptly to the Coordinator
    • Remain awake and alert during all shifts worked; (unless on a sleep shift) must be ready to respond at all times If you appear to be sleeping, we will assume you are sleeping and you will be held accountable, as such.

    General
    • Report time worked accurately, and punch in and out regularly via the ADP system
    • Ensure that company and client resources, including food and supplies, are used appropriately. Client and company food, drink and supplies, should not be used by staff.
    • Attend/take required training as determined by Rightway Home Care Service policy and your supervisor
    • Provide input at staff meetings
    • Become familiar with licensing standards in order to ensure compliance
    • Staff may be transferred to different departments (site locations) depending on the needs of the program, with or without the agreement of staff
    • Other duties as requested by supervisor

    Minimum Qualifications:
    • Must be age 18 or over
    • Must have a valid driver’s license for all positions that require transporting clients
    • Must have the ability to accurately complete documentation using grammatically correct English
    • Must have the ability to clearly verbally communicate with clients in English or ASL (depending on house)

    Other Job Related Requirements:
    • Must be willing to flex work schedule to accommodate staffing needs. Inability or unwillingness to flex work hours to accommodate the needs of the clients/agency may result in forfeiture of hours.
    • Must have the ability to complete required documentation and communicate effectively with clients.
    • Must be willing to immediately submit to a DHS Background Study including fingerprints. Background study checks may disqualify the employee from having contact with people receiving services, and thus would result in immediate termination.

  • Homemaking Specialist

    FUNCTION: The Homemaker provides household duties to those individuals requiring socialization and/or minimum guidance to assure environment and performs home management services within the client’s home.
    RESPONSIBILITIES:
    • Perform basic household tasks including cooking, laundry, cleaning, and maintenance
    • Plan and cook meals according to family’s personal preferences while keeping nutrition value in mind
    • Decorate homes to create an inviting environment for owners

    In-home Family Support Specialist: Provides supports to a person and his/her family (including extended family members) in the family’s home and/or in the community to enable the person to remain in or return to the home. This includes training of the person and family members to increase their capabilities to care for and maintain the person in the home.
    In-home family supports can be delivered in the person’s family home or community settings typically used by the general public.
    In-home family supports include assistance to develop, maintain or improve skills related to activities of daily living, such as:

  • Licensed Practical Nursing

    JOB SUMMARY:

    Performs a variety of professional nursing duties in the care of home care clients, complies with state rules and regulations, and follows established policies and procedures of the building.

    RESPONSIBILITIES:

    • Reports changes in client condition or other concerns about clients to the RN in a timely manner.

    • Initiates emergency procedures for clients and reports to RN.

    • Reviews client care plans developed by the RN and consults with RN about client conditions and their treatments.

    • Assists with implementation of physician orders.

    • Ensures accuracy and promptness in transcribing physician orders.

    • Carries out proper charting practices and procedures.

    • Assists with start up of services for new clients, hospital transfers and discharges as requested by RN supervisor.

  • Personal Care Assistant

    FUNCTION: To provide direct hands-on care to clients in order to assist each client reach and maintain his or her highest level of functioning.
    RESPONSIBILITIES:
    • Understand and adhere to information and precautions contained in individual clients’ Home Plan of Care;
    • Assist in scheduled client evaluations and assessments with other members of the care team.
    • Assist the client with exercise, ROM, ambulating, transfer activities as needed.
    • Assist the client with personal care activities and ADLs including bathing, skin care, back rub, hair care, nail care, dressing and undressing, feeding, oral hygiene, shaving, grooming, and bedtime care.
    • Take and record vital signs such as BP, temperature, pulse and respiration of clients as may be required from time to time.
    • Give catheter care to clients; collect urine and stool specimens, do diabetic urine testing, give suppositories and enemas; record intake and output as may be required sometimes.
    • Assist the client to and from the bathroom or in use of urinal or bedpan. Keep incontinent clients clean and dry.
    • Assist the client with self-administration of medications.
    • Enter or document client’s status and the care or service rendered into the clinical record.
    • Report any observed or noticeable changes in clients’ physical condition, attitude, reaction, appetite, to the supervisor promptly.
    • Report all client and employee incidents/accidents to supervisor immediately.
    • Report any unsafe client behavior such as unauthorized smoking, combative behavior, etc., to supervisor same day.
    • Maintain a clean, safe, comfortable and healthy environment for the client.
    • Observe safety precautions, including: wiping up wet floors, spills, and other falling hazards immediately. Report safety hazards such as frayed electrical cords, unsecure handrails, malfunctioning smoke and carbon monoxide detectors/alarms, defective equipments or environmental hazards to the supervisor immediately.
    • Use transfer belt whenever transferring and/or ambulating clients; use mechanical lifts (hoyer, century) whenever appropriate for transfers.
    • Use proper body mechanics while assisting clients to prevent avoidable injuries.

    SPECIFICS:
    • PCA must be in good physical condition, have a current chest x-ray or

    Must be at least 18 years of age and completed the training requirements as established by the
    Minnesota Department of Human Services OR must be 16 – 18 years old and have completed a home health aide competency evaluation.

  • Personal Support Specialist

    Personal Support: Specialist work with disabled adults to:
    • Achieve his/her full potential
    • Increase his/her independence
    • Meet community inclusion goals that are important to and important for the person and based on an assessed need.
    This service is provided in the person’s home or community.

    Personal support services relate to outcomes identified by the individual and their team when teaching and training is not necessary to achieve these outcomes, It is the least costly assistance to reasonably meet the person’s needs. Covered services include supervision, support or assistance with:
    • Activities of daily living (ADLs)
    • Accessing community services
    • Developing meaningful connections with community members
    • Establishing new relationships and nurturing existing ones
    • Participating in community activities of the person’s choosing.
    Services provided one-on-one with the person outside of his/her home must be provided in integrated community settings that enable the person to interact with people with and without disabilities to the fullest extent possible.

  • Registered Nursing (RN)

    A Registered Nurse is a graduate from an accredited school of Nursing, and must have a current and valid Minnesota RN license.
    Must also have a CPR Certification or get trained in CPR within 90 days of hire.
    Responsibilities include:
    The Registered Nurse will
    Assess the nursing needs of clients, residents, or students, developing individualized nursing plans to meet those needs through direct nursing intervention and/or delegation of specific nursing tasks to nursing assistants.
    Evaluate the effectiveness of the plan.
    Coordinate and review the work of nursing staff members.
    Be able to work independently with minimal supervision from higher level nursing personnel.

    Perform nursing direct treatment and procedures.
    Or may delegate and review nursing tasks performed by other staff.
    Provides assessment of client’s or resident’s skills and suggests appropriate nursing intervention.

    Reports and interprets observations of client’s or resident’s behavior.
    Or must possess critical thinking skills: Ability to prioritize multiple tasks.
    Or must possess strong communication and verbal skills.
    Communicate to unit director on nursing or health care problems.
    Teach self-help skills to residents or clients.
    Act as role model for clients or residents.
    Maintain client or resident charts, records and confidentiality.
    Provides informal training and instructions such as reviews and discusses nursing policies, procedures and techniques.

    Or may provide informal training and instructions such as reviews and discusses nursing policies, procedures and techniques.

    Participates in group activities and advises on the group process.
    Or must possess the ability to work intimately with clients and families throughout the life continuum and be a strong patient advocate.
    Or must possess the ability to establish effective relationships with healthcare team members, managers and leaders along with key persons including families; must be a team player.
    Perform related duties as required or delegated.

  • Respite Specialist

    Respite Specialist is an individual that would be responsible for the care of a client while the primary caregiver is out of the home. Short-term care services provided to a person when the person’s primary caregiver is absent or needs relief from caregiver duties.
    The time frame may vary from shifts to 24 hours a day. Clients are able to receive up to 30 consecutive days per respite occurrence. The length of the person’s stay must be documented in the support plan.
    Respite provides the level of supervision and care that is necessary to ensure the person’s health and safety.
    Which includes but not limited to:
    • Med administration
    • ADL’s
    • Meal preparation
    • Light homemaking
    • Laundry
    • Transportation to Respite activities/appointments

    Providers licensed under 245D must report all uses of controlled procedures, emergency use of manual restraint and prohibited procedures according to Minn. Stat. §245D.06, subd. 5 to DHS via the Behavioral Intervention Report Form, DHS-5148.

    Requirements for this position:
    • Must pass the Minnesota Department of Human Services Background Study.
    • Valid Driver’s License, Full coverage insurance, Reliable vehicle.
    • Willing to drive in Hennepin, Ramsey, Anoka counties. Caseloads are assigned generally in approximate 30 mile or less, radius.
    • Must be very organized, detail orientated, self-starter.Must be computer literate, able to read and send emails from phone and computer. Able to use phone apps for mileage and time keeping.
    • Possess problem solving skills, able to multi-task, and follow through with all tasks given in required time and to completion.
    • Must have excellent work ethics. Must be trustworthy, reliable and willing to work entire shifts fulfilling all obligations.
    • Must be flexible and team player. Willing to make changes in schedule to assist Supervisor when asked.
